and addressing the needs of various clients. Professionals are free to download and use any of these resources for non-commercial

Children's Behavior Profile
The Children's Behavior Profile serves as a useful initial screening device for a wide range of childhood concerns, including
depression, anxiety, ADHD, oppositional defiant disorder, conduct disorder, obsessive compulsive disorder, bipolar disorder,
autistic spectrum disorders, adjustment disorder, substance abuse, and school and learning problems. The screening instrument
is designed as a behavior rating scale completed by parents. Use of this instrument is a standard part of my intake assessment
for all children and teens, providing me with a useful overview of the mental health status of the referred child. Important

Sensory Sensitivity Checklist
I developed this checklist as an initial screening device for Central Auditory Processing Disorder (CAPD) and Scotopic Sensitivity
Syndrome (SSS), two disorders that often mimic ADHD symptoms and should be ruled out when assessing for ADHD. Professionals
are asked to e-mail me for the scoring key for this instrument (use request form found below).
Mood Assessment Scale
The Mood Assessment Scale serves as an initial screening device for early onset (childhood) bipolar disorder. The rating
scale is completed by parents. Scoring key included with rating scale. Items included in this instrument were derived from
the work of Papolos and Papolos (The Bipolar Child, Broadway Books, 1999).

This checklist is useful both for parents and professionals when addressing children's learning problems and needs.
Checklist for Monitoring Medication Progress: Client Form
This form is useful in evaluating the efficacy of current medcation treatment. It can be used by adult clients, or by parents
evaluating their children's medication progress. I often forward the results of these ratings onto prescribing physicians
to assist them in their treatment decisions. Professionals who wish to use this form are free to replace my letterhead
with their own.
